The Community Health Management (CHM) Hub®
The CHM Hub allows us to pinpoint areas where essential services are missing, like nutrition, transportation, fitness, or pharmacy services. Then, we can make detailed, personalized recommendation plans to companies who may have SDOH obstacles, so they can be proactive about addressing these issues.
